Next Steps for Students
Once your application has been submitted and approved, you will receive a confirmation email from Flowers Hospital. After your confirmation email has been received, you must complete the required forms and documentation below before your clinical rotation or internship will be scheduled.
Submit all forms and documentation together as a packet to the human resources department at Flowers Hospital. The human resources department is located on the 2nd floor of the North tower in the Doctors Center building at Flowers Hospital.

The following documentation will also be required:
- School documentation of current enrollment in the program listed on your application
- Examples: a letter from your college/university or a current transcript or class schedule
- A copy of your government-issued photo ID
- A copy of your current BLS certification:
- American Heart Association – Healthcare Provider
- American Red Cross - Healthcare Provider
Cards must say Healthcare Provider
- Immunization records/titers (Please review our health requirements to verify that your current immunizations/titers will meet our needs.)
- The following two documentation requirements should be obtained from your college/university.
- Current 10 panel drug screen report (within the past 12 months)
- Current background report that includes a county, state and federal criminal records search (within the past 12 months)
When the required documents above have been submitted and approved by human resources, a second email will be sent to you, your preceptor and the department director confirming the receipt of the required documents. The required documents must be received by the human resources department at least one week prior to your first day of clinical rotations or internship.
On your first day, you must report to the human resources department to present your government-issued photo ID and be given a Flowers Hospital student badge. You will not receive a badge or perform any student assignments until all required documents and a picture ID have been presented. You will also be required to return your badge on your last day of clinical rotations.
